Medical-Dental Integration (MDI) occurs in many forms.  We primarily focus on two: program implementation and data collection.  We have compiled a list of activities involving MDI that we discovered through research and outreach but this does not constitute an exhaustive list.  If you know of a program that should be listed below, please contact us!

Colorado – “The CO MDI Project integrates full scope dental hygiene services into medical settings by adding dental hygienists to medical care teams. Patients visiting their medical provider can also see a hygienist in a convenient and familiar setting.”

Connecticut

Connecticut – “The US Department of Health and Human Services (HHS) Health Resources and Services Administration (HRSA) funds state level projects aimed at improving maternal and infant oral health. Connecticut’s Perinatal and Infant Oral Health Quality Improvement Project (PIOHQIP) was funded in the first round of the five-year HRSA grant period (2013-2017). The Connecticut Department of Social Services (DSS) conducts this project with its administrative services contractor, the Connecticut Dental Health Partnership (CT DHP). PIOHQIP in Connecticut is focused on improving oral health among pregnant women in low-income families with HUSKY Program (Medicaid and CHIP) coverage.”

Tampa, FL – Suncoast Community Health Centers (FQHC; 501(c)(3) non-profit organization) has 12 locations including 7 pediatric medical clinics (Brandon, Ruskin, Thonotosassa, Palm River, Tom Lee, Wimauma, and SCHC).  They also have 5 OB/GYN clinics (Ruskin, Palm River, Tom Lee, Plant City, and SCHC Women’s Care of Lakeland).  Their medical-dental integration program incorporates a dental hygienist into the pediatric program.  According to the YouTube video, their program started July 2017.

Maryland

Maryland – “Strategies to Improve Oral Health Care Utilization of Pregnant Women in Maryland: Partnering of Title V and PIOHQI” (link to slides)

From Slides:

  • The team worked “with two FQHCs (one urban, one rural) in Maryland to train OB/GYN providers on perinatal and infant oral health”
  • The team “explored ways to improve utilization of oral health care for pregnant women (by implementing an) oral health screening appointment prior to prenatal care (to integrate) oral health into OB/GYN prenatal care.”  The team asked:
    • “Is oral health part of prenatal care? If yes, how and what? If not, why?
    • If woman does not have a dental home, does OB/GYN provider refer to a dental provider and what is the process? Is there a warm handoff and follow-up?”
  • The team “developed and disseminated state perinatal and infant oral health guidelines to dental and OB/GYN providers” (presentation | slides)

Minneapolis, MN – “People’s Center is a nationally recognized non-profit community health center serving diverse communities in south Minneapolis and surrounding communities. People’s Center operates a network of three clinics and accepts all patients regardless of their ability to pay.”  They have a pretty unique centering-style pregnancy program called “Hooyo” that “was co-created in partnership with Somali mothers and other community providers including Early Childhood and Family Education (ECFE), prenatal yoga instructors, Women, Infant and Children (WIC), massage therapists, nutrition and pediatric providers who help us deliver these quality services. To learn more, please contact Christine Rangen at rangenc@peoples-center.org”.  Their “KidSmile Pediatric Dental Project utilizes (a) mobile dental unit to travel to child care centers, elementary schools and community centers to offer complimentary dental sealants, oral health screenings, fluoride vanishes and oral health education to children ages 12 months to 14 years of age.”

Currently Researching: Finger Lakes Community Health (Mary Zelazny)

Albany, NY – “With support from an NYSHealth grant, Albany Medical Center has co-located pediatricians at upstate WIC sites to provide oral health services to Medicaid-eligible preschool-age children in areas where access to dental care is limited. At the sites, mothers receiving WIC benefits also are able to bring their infants and children (up to five years old) to the on-site pediatrician for a basic dental screening and a fluoride varnish application—an important tool in cavity prevention.”  This project was started by Melinda (Mindy) Clark, a pediatrician with Albany Medical Center and initiator of WIC Smiles.  Click here for more information

Tulsa, OK – Sean Costello, DDS has a “Kid’s Dental” practice in Tulsa.  He is a “SoonerCare” provider meaning his practice takes Oklahoma Medicaid.  He has a WONDERFUL website devoted to perinatal care information (link here).

Pennsylvania – “Keystone First has created the Circle of Care value-based program for participating OB/GYN providers. This program aims to integrate dental and medical services within the scope of the dental practice while understanding the adverse effects of certain medical conditions on oral health. Although the procedures to accomplish this integration are not usually within the day-to-day operations of a dental office, it is important for primary dental care providers (PDPs) to be knowledgeable about each patient’s medical history as it relates to his or her oral health and to discuss these issues with the patient.”

“For pregnant members, an incentive payment will be made to the providers when a dental claim is noted and paired with an OB/GYN claim within 60 days prior to the date of the initial dental appointment. Once an eligible member receives an initial exam that corresponds with a specific dental (D) code, a 60-day look back will be initiated. If an OB/GYN claim is identified within this 60-day time frame, an incentive payment of $25 per qualifying member will be paid out to the OB/GYN. The goal is for the physician and staff to have oral-health-focused conversations relating to the physical health conditions, and refer the members to dentists within the network participating in Circle of Care.”

Texas – “The Texas Repository of Oral Health Data for Evaluation and Outcomes (ROHDEO) is a statewide database of oral health indicators contributed by community dental programs in Texas… The Basic Screening Survey (BSS) is a limited oral evaluation promoted by the Association of State and Territorial Dental Directors. There are three modules of the BSS: preschool age children, school age children, and adults. Data from any module can be collected and contributed to ROHDEO. Basic demographic data (county, gender, race/ethnicity, etc.) and data on access to dental care (last dental visit, dental insurance status, etc.) are all collected in ROHDEO.”

Milwaukee, WI – The medical-dental integration (MDI) programs at Sixteenth Street Community Health Centers appear to be emerging and relatively new.  They have some information about their women’s health and pregnancy services listed on their website (link here).  Patti Bacon DeQuardo is a good resource for more information at this site.

Ascension also has a network that conducts MDI in Wisconsin (link here).  They have an 11-chair dental clinic, a school based sealant program, a 2-chair mobile dental clinic and now have hygienists working in medical clinics and with OB patients.  Jennifer Jon, Clinic Coordinator at the Dental Clinic, is a good resource for more information at this site.
